DirectLink patient placement service provides Case Managers and
Discharge Planners with access to over 10,000 skilled nursing,
sub-acute, and rehab beds. DirectLink helps you place:
- the RIGHT Patient
- in the RIGHT Facility
- with the RIGHT services
- and the RIGHT coverage
One phone call gives the Case Manager or Discharge Planner access
to a care specialist who can expedite the search for and location of
the most appropriate post-acute setting for the patient.
